SMS/text message communications, you agree to be bound by these Terms and Conditions. If you do not agree, please do
not use our services.

2. Services
Andalusia Therapy Group provides therapy and mental health services. Our communications services include appointment
scheduling, appointment reminders, account notifications, and customer care via telephone, email, and SMS text messaging.

3. SMS/Text Messaging Terms
By providing your mobile phone number and opting in to receive text messages from Andalusia Therapy Group, you agree to
the following:
You consent to receive recurring SMS messages related to appointment reminders, account notifications, and customer
care
Message frequency varies based on your appointment schedule and account activity
Standard message and data rates may apply depending on your mobile carrier and plan
You may opt out at any time by replying STOP to any message
You may request help at any time by replying HELP to any message
SMS is not used for marketing or promotional purposes
Your mobile number and SMS consent information will not be shared with third parties for promotional or marketing
purposes

4. Consent to Receive Messages
Your consent to receive SMS messages is obtained when:
You verbally agree during a phone call or in-person visit after being informed of the SMS program details
You provide your phone number on an intake form that includes SMS consent language
You opt in through our website or patient portal
After opting in, you will receive a confirmation message: "You have agreed to receive SMS updates from Andalusia Therapy
Group. Msg freq may vary. Std msg & data rates apply. Reply STOP to opt out, HELP for help."
5. Opt-Out
You may stop receiving SMS messages at any time by texting STOP to our number. Upon receipt of your STOP request, you
will receive a final confirmation message: "You have been unsubscribed and will no longer receive messages from us. Reply
HELP for assistance."
Opting out of SMS does not affect your ability to receive healthcare services from Andalusia Therapy Group.

6. Help
For assistance with SMS messaging, reply HELP to any message or contact us directly. You will receive: "Andalusia Therapy
Group: For help, reply HELP or contact us at (334) 343-6129. Msg frequency varies. Msg & data rates may apply. Reply STOP
to opt out."

7. Privacy
Your privacy is important to us. Please review our Privacy Policy for information on how we collect, use, and protect your
data. SMS messages will not contain Protected Health Information (PHI) unless you have provided written opt-in consent in
accordance with HIPAA regulations.

8. Patient Responsibilities
Provide accurate and up-to-date contact information
Notify us of any changes to your phone number
Arrive on time for scheduled appointments
Provide at least 24 hours notice for appointment cancellations

9. Limitation of Liability
Andalusia Therapy Group is not responsible for:
Delays or failures in SMS delivery caused by mobile carriers or network issues
Charges from your mobile carrier for receiving text messages
Any actions taken based on information received via SMS

10. HIPAA Compliance
Andalusia Therapy Group operates in compliance with the Health Insurance Portability and Accountability Act (HIPAA). All
patient information is handled in accordance with HIPAA Privacy Rule and Security Rule requirements. A Notice of Privacy
Practices is available upon request.

11. Modifications
We reserve the right to modify these Terms and Conditions at any time. Updates will be posted on our website with a revised
effective date. Continued use of our services constitutes acceptance of any changes.
12. Governing Law
These Terms and Conditions are governed by the laws of the State of Alabama. Any disputes arising from these terms shall
be resolved in the courts of Covington County, Alabama.
